The lid. A lift-off lid opens as a reveal rather than an unwrapping, and the gap between lid and base is the detail customers read as quality - too loose and it rattles, too tight and it drags. That gap is set at dieline stage.
In folding carton stock, yes - both pieces fold flat and are assembled at packing. In rigid board it arrives assembled, which protects the edge quality but takes considerably more storage and freight volume.
An insert, in almost every case. A two-piece box has no flaps to restrain the contents, so anything that can move will move. A punched tray or cut insert both protects the product and sets the presentation when the lid comes off.
